Common Causes
Diabetic foot complications develop from the combined effects of long-term high blood sugar on both the nerves and blood vessels supplying the feet.
- Long-standing or poorly controlled diabetes, the underlying driver of nerve and vascular damage.
- Reduced sensation (neuropathy) allowing minor injuries to go unnoticed and untreated.
- Poor circulation slowing the body's ability to heal wounds and fight infection.
- Ill-fitting footwear causing unnoticed pressure injuries or rubbing.
- Foot deformities altering pressure distribution and increasing risk at specific points.
- Delayed presentation - complications are often more advanced by the time they are noticed, due to reduced sensation.
Signs and Symptoms
- Numbness, tingling, or reduced sensation in the feet
- A wound or ulcer that is slow to heal or was not initially noticed
- Swelling, warmth, or redness suggesting infection
- Changes in foot shape, sometimes indicating Charcot foot
- Skin changes, dryness, or cracking that can predispose to injury
- Discoloration or coldness suggesting reduced blood flow
Diagnosis
Assessment includes checking sensation, circulation, and skin condition, along with direct examination of any wound present. Imaging or vascular studies may be used to assess bone involvement or blood flow when a wound is not healing as expected, and blood tests help confirm whether infection is present.
Treatment Approach
Treatment depends on what is found - wound care and infection control for an ulcer, offloading pressure from at-risk areas, and addressing any underlying circulation problem where possible.
