Bone Infection and Tuberculosis Treatment
Treatment for bone infections including bone TB.
Bone infections occur when bacteria or, in the case of bone tuberculosis, the TB bacterium reaches the bone and causes inflammation and tissue destruction. These infections can develop after surgery, an open injury, or spread from elsewhere in the body, and need to be identified and treated properly to prevent lasting damage to the bone and surrounding joint.
Types
Acute Osteomyelitis
A bone infection that develops rapidly, usually with fever, pain, and swelling over the affected bone.
Chronic Osteomyelitis
A long-standing bone infection that has persisted or recurred, sometimes following inadequately treated acute infection.
Bone Tuberculosis (Skeletal TB)
Infection of the bone or joint caused by the tuberculosis bacterium, most commonly affecting the spine.
Post-surgical Infection
Infection introduced during or after an orthopaedic surgical procedure.
Post-traumatic Infection
Infection following an open fracture or injury where bacteria entered the wound directly.
Common Causes
Bone infections generally reach the bone in one of a few ways - through the bloodstream, through direct contamination from an injury or surgery, or by spreading from a nearby infected tissue.
- Open fractures - injuries where broken bone is exposed to the outside environment, allowing bacteria direct entry.
- Surgical contamination - bacteria introduced during an orthopaedic procedure, though modern sterile techniques make this uncommon.
- Bloodstream spread - bacteria travelling from an infection elsewhere in the body to settle in a bone.
- Untreated pulmonary tuberculosis - TB bacteria spreading from the lungs to bone, most often the spine.
- Weakened immunity - conditions or medications that reduce the body's ability to fight off infection increase susceptibility.
- Diabetes - poorly controlled diabetes significantly increases the risk of bone and soft tissue infections, particularly in the feet.
Signs and Symptoms
- Persistent pain over the affected bone that does not improve
- Swelling, warmth, or redness over the area
- Fever, particularly in acute infections
- A wound that drains fluid or does not heal
- Gradual, unexplained back pain in cases of spinal TB
- General fatigue and weight loss, especially with bone tuberculosis
Diagnosis
Diagnosis combines blood tests, X-rays, and often an MRI or CT scan to assess the extent of bone involvement. A biopsy or fluid sample from the affected area is usually needed to identify the specific bacteria or confirm tuberculosis, since treatment differs significantly depending on the organism involved.
Treatment Approach
Treatment depends on the type and extent of infection. Bacterial osteomyelitis typically requires a prolonged course of targeted antibiotics, often alongside surgery to remove infected or dead bone tissue. Bone tuberculosis is treated with a extended course of anti-tubercular medication, sometimes combined with surgery if there is significant bone destruction or spinal instability. Chronic or long-standing infections may need multiple stages of treatment to fully clear. Dr. K. G.
