ACL Injury Treatment

ACL injuries are among the most significant ligament injuries in sport, commonly occurring during activities involving sudden pivoting, deceleration, or awkward landings. Treatment decisions depend on the severity of the tear, the patient's activity level and goals, and whether the knee remains stable enough for daily activity without surgery.
Types
Partial ACL Tear
The ligament is stretched or partially torn, sometimes manageable without surgery depending on residual stability.
Complete ACL Tear
The ligament is fully torn, most often requiring reconstruction for patients wishing to return to pivoting sports.
ACL Tear with Combined Injury
An ACL tear occurring alongside meniscus or other ligament damage, common in higher-energy injuries.
Common Causes
ACL injuries typically occur during a specific sporting incident involving sudden changes in movement or direct impact to the knee.
- Sudden pivoting or cutting movements, common in football, basketball, and similar sports.
- Landing awkwardly from a jump.
- Sudden deceleration while running.
- Direct contact or collision to the knee during play.
- Pre-existing knee looseness or muscle imbalance increasing susceptibility.
Signs and Symptoms
- A popping sensation at the moment of injury
- Rapid swelling within hours of the injury
- A feeling of the knee being unstable or giving way
- Difficulty continuing the activity immediately after injury
- Ongoing instability during sport or daily activity in an untreated tear
Diagnosis
Diagnosis is based on clinical stability testing of the knee, confirmed by an MRI, which also identifies any associated meniscus or cartilage damage that commonly occurs alongside ACL tears.
Treatment Approach
Treatment depends on the severity of the tear and the patient's activity goals - some partial tears in less active patients are managed with physiotherapy and bracing, while complete tears in athletes wishing to return to pivoting sports generally need surgical reconstruction.
